Bring the following information to the parish CCD & Confimation Office at St Patrick Church.

1. Birth Certificate

2. Original Baptismal Certificate*

                     *Copies or emailed certificates will not be accepted. 

Please fill out the below Medical Consent Form completely and don't forget to sign it.
